HMO

A Health Maintenance Organization (HMO) is a type of insurance policy that limits your provider network to local facilities and usually requires a referral to see specialist. If you have an HMO Plan you should be sure to stay in-network or it could cost you.

PPO

A Preferred Provider Organization (PPO) is a type of health plan where you can use any doctor, hospital or provider. However, if you use "in-network" doctors you will pay much less for care. 

These networks are huge so you will have a lot of doctors to choose from. 

POS

A Point Of Service Plan (POS) have a smaller network than a PPO plan; however, their costs for in-network care are usually lower, like that of an HMO.  You must choose a primary care provider and any other provider from within the network of doctors. You must also get a referral to see a specialist as well.

EPO

An Exclusive Provider Organization (EPO) only covers you if you use an in-network doctor and while you won't need a referral to see a specialist, you will pay full cost if you use an out of network doctor.  Depending on the company you choose this plan might require you to pick a primary care physician.

What Is The Difference Between HMO And PPO? 

The differences between an HMO (Health Maintenance Organization) and a PPO (Preferred Provider Organization) is that an HMO is going to have lower monthly premiums compared to a PPO. However, an HMO will require you to stay in-network for your providers. With a PPO you can visit doctors both in and out of network and it doesn't require you to have a referral to see specialists.

Your health insurance coverage will help with most hospital and doctor visits, mental health, wellness care, preventative care, prescriptions and medical devices. Each of these things are covered under different parts of your policy which we discuss below: 

Deductible

Your deductible is how much you must pay out of pocket (per year) before the insurance company will start covering you (co-insurance). Deductibles can range from $0 to $10,000. The lower the deductible the higher the monthly premium. 

Co-Insurance

Once your annual deductible has been met your co-insurance will kick in to assist you with covering health care costs.  Usually you see this as 80/20 or 70/30.  The first amount is covered by the insurance company (80%) and the second amount is covered by you (20%)

Co-Pay

Your co-pay is a set amount you will pay to visit your doctor. They can range depending on if you are visiting your primary doctor or a specialist. This amount is separate from your co-insurance and usually must be paid on a per visit basis.

Out Of Pocket Maximum

Each policy will have an out of pocket annual maximum.  This is the most you can spend out of pocket in a year before you are essentially completely covered. These amounts will depend on your deductible, co-insurance and prescription drugs.

Prescription Drugs

Usually prescription drugs are going to be covered based on formulary tiers. Like Tier 1, Tier 2, Tier 3 and so on. The higher the tier the higher the cost for the prescription . You could also have a separate prescription drug deductible from your primary plan. 

Preventative Care

Most policies come with a free annual checkup when you first get your policy and then 1 free check-up yearly.  Some insurance companies are focusing more on preventative care in the beginning to help people have better long-term health care. 

What Does Health Insurance NOT Cover?

Health insurance does not cover cosmetic procedures, fertility treatments, off-label prescriptions or new technologies and products. Usually because most cosmetic procedures and fertility treatments are considered elective by the insurance companies. And, since prescription drugs are tested and approved for certain disorders, insurance companies won't cover your prescription costs if your disorder isn't listed on the "label."

Your Age

Like most insurance, your age will help determine what your premiums will be. The younger you are when you purchase health insurance the more affordable it will be. 

Gender

All things being the same, women will pay higher premiums than men because women usually visit the doctor more often and take more prescriptions than men. 

Tobacco Use

Tabaco use is probably the one item on the list that will give you the largest increase in your premiums. Upwards to 60% or more per month when it comes to health insurance.

Health

Your health is the one factor that makes everyone's premiums different. While you can no longer be declined for insurance due to pre-existing conditions your premiums can be higher.

Your Deductible

The lower your deductible the higher your monthly premium.  Premiums can vary wildly depending on the amount of your deductible.  If you want the lowest premium, you will need a higher deductible. 

Your Co-Insurance

The more coverage or co-insurance you purchase the higher your premiums. An 80/20 policy is more expensive than a 60/40 policy, but it also gives more coverage. It's best to pick the policy based on needs and affordability.
