How It Works

  • True Own-Occupation Pays if you can no longer practice your medical specialty — even if you keep working in another role. This is the clause the whole policy turns on.
  • Priced by Occupation Class Carriers sort specialties into 4A-3A-2A classes that set your base rate. Quoting through a physician-focused broker gets you the class you actually belong in.
  • Lock In Your Rating Early Issue age and health rating are set at application. Residents and fellows buy the same policy for $80-$175 a month and keep that pricing for decades.
  • Benefits Arrive Tax-Free When you pay premiums with after-tax dollars, the monthly benefit is not taxed — unlike most employer-paid group LTD.

What to Look For

What a Good Physician Policy Has
  • True own-occupation, specialty-specific, for the full benefit period
  • Non-cancellable AND guaranteed renewable to age 65
  • Residual / partial disability benefit included, not sold separately
  • Future purchase option so coverage grows with your income
Red Flags to Avoid
  • "Any occupation" language, or own-occupation that flips after 2 years
  • Guaranteed renewable only — the carrier can still raise your premium
  • Treating hospital group LTD as enough on its own
  • Association plans that can be re-rated or cancelled as a block

What Is Physician Disability Insurance?

Physician disability insurance is individual long-term disability coverage written for the way doctors actually earn a living. It replaces a share of your income — usually 60-70% of gross — if illness or injury stops you from practicing your specialty.

It is a distinct product from generic disability insurance for two reasons. First, the definition of disability is specialty-specific: a policy that pays a surgeon who can no longer operate is doing something a standard "any occupation" policy will not. Second, the pricing is set by occupation class, so a dermatologist and a neurosurgeon buying the same benefit pay very different premiums.

For most physicians the individual policy sits on top of a hospital or group plan rather than replacing it. Group LTD covers base salary; the individual policy covers the rest of what you earn, and pays tax-free.

How Much Does Physician Disability Insurance Cost?

Plan on 1% to 4% of your annual income — roughly $150 to $500 per month for most doctors. Career stage is the first cut:

Career StageTypical Monthly Premium
Medical student$30-$80 (guaranteed insurability rider)
Resident / fellow$80-$175
Early attending (28-35)$150-$350
Mid-career attending (36-45)$250-$500

Two dials then move the number in either direction. The benefit period — "to age 65" costs more than a 5- or 10-year period, and for a physician protecting decades of earning power it is almost always the right call. And the elimination period — 90 days is the standard sweet spot, and stretching to 180 days trims 10-15% off the premium if you hold enough cash to bridge the gap.

Physician Disability Insurance Cost by Specialty

Specialty is the single biggest pricing factor, because carriers sort medical specialties into occupation classes (usually a 4A-3A-2A structure) that set the base rate before anything else is considered. A surgeon uses her hands in ways a psychiatrist never does, and the claims data reflects it.

Approximate monthly premiums for a 35-year-old physician buying $10,000/month in benefits, 90-day elimination period, benefits to age 65:

SpecialtyApprox. Monthly Premium
Neurosurgery$420-$570
Orthopedic surgery$400-$550
General surgery$380-$520
Anesthesiology$350-$480
Emergency medicine$320-$450
Internal medicine$170-$270
Family medicine$160-$260
Pediatrics$155-$255
Psychiatry$150-$250
Dermatology$145-$235

If you are a psychiatrist being quoted $450 a month somewhere, that is a signal to get a second set of quotes — not a reflection of what your class should cost.

Own-Occupation vs Any-Occupation for Physicians

This is the clause that decides whether the policy is worth owning.

  • True own-occupation pays the full benefit when you can no longer perform the material duties of your medical specialty, even if you earn income doing something else. The surgeon with a hand tremor collects while teaching.
  • Modified own-occupation pays own-occupation benefits only while you are not working elsewhere, or only for a stated window (often 2-5 years) before shifting to any-occupation.
  • Any-occupation pays only if you cannot work in any job your education and experience qualify you for. For a physician this is close to unclaimable.

Expect true own-occupation to cost 10-20% more than a modified definition. For a procedural specialist, that premium is the reason to buy the policy at all.

Which Riders Are Worth Paying For?

Riders add roughly 5-15% each. Three earn their keep for most physicians:

  • Future purchase option (guaranteed insurability). Lets you increase monthly benefit as income grows with no new medical underwriting. Essential if you buy during residency, when the benefit you can qualify for is a fraction of your eventual income.
  • Residual / partial disability. Pays a proportional benefit when you return at reduced hours or reduced income. Most real claims are partial rather than total, which makes this the rider that actually gets used.
  • Cost-of-living adjustment (COLA). Indexes the benefit once a claim begins. Worth it on a "to age 65" policy bought young, where a claim could run thirty years.

Also confirm the policy is non-cancellable, not merely guaranteed renewable. Guaranteed renewable means the carrier must renew you but can raise premiums across the whole class; non-cancellable locks the premium as well as the coverage.

How to Buy Physician Disability Insurance

Underwriting is stricter than life insurance — carriers examine occupation, income, and medical history in detail. The process:

  1. Check what your hospital or group plan already covers, and what it excludes (call pay, bonus, and partnership income are the usual gaps).
  2. Decide the benefit amount, elimination period, benefit period, and definition of disability before you quote, so the quotes are comparable.
  3. Quote at least three carriers on identical terms. Guardian, MassMutual, Principal, and Ameritas are the four physicians most often end up comparing.
  4. Ask specifically whether own-occupation is true or modified, and whether the policy is non-cancellable. Both answers should be in writing.
  5. Complete the application and the medical exam. Underwriting typically takes 4-8 weeks.

Buy early. Issue age and occupation class are set at application, and a diagnosis between now and then can make the policy either more expensive or unavailable.

Frequently Asked Questions

How much does physician disability insurance cost?

Typically 1-4% of your annual income, which works out to roughly $150-$500 a month for most doctors. A 30-year-old family medicine attending buying solid own-occupation coverage lands around $150-$250; a 35-year-old surgeon in a high-risk specialty is closer to $350-$500 for the same benefit amount.

What does "true own-occupation" mean, and why does it matter for doctors?

True own-occupation pays your full benefit if you can no longer perform the duties of your medical specialty, even if you go on to earn money doing something else. "Any occupation" only pays if you cannot work at all. For a surgeon who develops a tremor, that difference is the entire policy — she can still teach, and only own-occupation keeps paying.

Should I buy disability insurance as a resident or fellow?

Yes, and it is the cheapest decision in this whole process. Residents and fellows pay roughly $80-$175 a month, medical students $30-$80 with a guaranteed insurability rider. Your issue age and health rating lock in at application, so buying before your income and your health change is what keeps the price down for the next thirty years.

Is my hospital group LTD enough?

Usually not. Group plans typically replace 60% of base salary only — no call pay, no bonus, no partnership distributions — and the benefit is taxable when the employer paid the premium. Most physicians layer an individual own-occupation policy on top and treat the group plan as the floor.

How much monthly benefit can I buy?

Carriers cap coverage at about 60-70% of gross income. Every additional $1,000 of monthly benefit adds roughly $20-$40 to the premium depending on your specialty class and age. Insuring 100% of income is not offered — carriers deliberately leave an incentive to return to work.

Which specialties pay the most?

Occupation class drives the base rate. For a 35-year-old buying $10,000/month with a 90-day elimination period and benefits to age 65: neurosurgery runs roughly $420-$570 and orthopedic surgery $400-$550, while dermatology sits around $145-$235 and psychiatry $150-$250. Anything procedural prices higher because the claims data says so.

Are disability benefits taxable?

If you pay the premiums yourself with after-tax dollars, benefits arrive tax-free. If your employer paid the premium and did not include it in your income, benefits are taxable. That is a large part of why a $10,000/month individual policy is worth more than a $10,000/month group benefit.
